DENTAL PLAN
The Encore Dental insurance plan can reduce customers’ dental bills, no matter what dentist they see. Having dental coverage is one of the greatest factors in determining whether a person visits a dentist. Costs vary depending on the region and plan selected. Premiums are guaranteed for the first year and the insured cannot be singled out for a rate change. Premiums cannot increase more than once in a 12 month period. (COMING Soon!)
TERM LIFE INSURANCE
The simplification of the underwriting process allows consumers to purchase valuable and needed term life without the hassles of medical exams and the prolonged wait times (from 3-12 weeks) of traditional underwritten products. Offered by Stonebridge Life Insurance Company premiums vary by age, gender and benefit amount, Coverage terminates at age 65; guaranteed as long as premiums are paid. (COMING Soon!)
PRESCRIPTION DRUG CARDAfter satisfaction of the applicable co-pay amount shown on the plan summary, the plan will pay the applicable benefit when a Covered Person has a prescription filled or refilled by a Pharmacist. Benefits will be paid up to the applicable annual maximum as shown in the plan summary.
With access to the AmWINS Rx network, the insured will still receive the benefit of the prescription drug discount card even after the annual benefit is exhausted. (COMING Soon!)
SMALL GROUP EMPLOYER EMPOYEE BENEFIT TRUST
